Ten Warning Signs of Nursing Home, Assisted Living Facility, and Group Home Abuse and Neglect

When you entrust a nursing home, assisted living facility, or group home with the care of someone you love, you expect compassionate, professional treatment. Unfortunately, that trust is too often betrayed. Abuse, neglect, and even wrongful death are far more common in Florida’s long-term care system than most people realize—and the warning signs aren’t always obvious.

1. Unexplained Injuries
Bruises, cuts, burns, or fractures that staff cannot adequately explain should raise immediate concern. These injuries may indicate rough handling, falls due to neglect, or even physical abuse.
2. Bedsores (Pressure Ulcers)
Bedsores are a clear indication that your loved one may be left in the same position for long periods—often in unsanitary conditions. These painful sores are entirely preventable with proper care.
3. Sudden Weight Loss or Signs of Malnutrition
If your loved one is losing weight rapidly or appears dehydrated, they may not be receiving adequate food, water, or medical attention.
4. Poor Hygiene or Unclean Conditions
Your loved one should always be clean, dressed appropriately, and living in sanitary conditions. Dirty rooms, strong odors, or soiled clothing are signs of neglect.
5. Emotional Withdrawal or Depression
Emotional changes can signal more than age-related decline. If your loved one seems scared, withdrawn, or depressed, especially around staff, it may point to abuse, isolation, or mistreatment.
6. Fearfulness or Anxiety Around Staff
Pay attention to how your loved one reacts when certain staff members enter the room. Fear or hesitation may indicate verbal, emotional, or physical abuse.
7. Sudden Changes in Medication or Overmedication
If your loved one appears overly sedated or if medications have changed without a clear medical reason, this could be a sign of chemical restraint—an illegal and abusive practice.
8. Staff Refuses Unsupervised Visits
If the facility prevents you from visiting privately or makes excuses to avoid letting you see your loved one alone, that’s a major red flag. What are they trying to hide?
9. Missing Personal Items or Financial Irregularities
Elder financial abuse is also very common. Watch for missing belongings, unexplained withdrawals, or even unexpected changes to financial accounts or estate planning documents.
10. Gut Instinct
Sometimes, you just know something is wrong. If your instincts tell you your loved one is not safe, trust them. You are their best advocate.
